Q1: Identify all the people who you think were at least partially responsible for the death of Bart Matthews, and why you think so?

Answer
For me, i think the responsible for the death of the operator, Bart Matthews are: FIRST, himself because he violated each and everyone of Shneiderman's rule. Several of these violations were directly responsible for the accident which resulted to his death. SECOND, Randy Samuels, a Silicon Techtronics programmer, because according to an investigation into the cause of the accident led authorities to the conclusion that a software module, written and develop by Randy Samuels, was responsible for the erratic and violent robot behavior which in turn lead to the death of Bart Matthews. THIRD, is the interface designer because he/she also violated the Shneiderman's "Eight Golden Rules" and he/she didn't have much experience with user interface technology because in an application where speed was of the essence, especially when operator safety was at issue, the use of a keyboard for all menu selection task (because the console had a keyboard, but no mouse) was an extremely Difficult.
